Sports Physicals

We believe it is important for your child to be active AND safe, so we are here to help them do just that!  If you have already had a wellness visit within the calendar year, we can complete your sports physical form with the information from that visit.  We may require a nurse-only visit to get specific vital signs required for a sports physical depending on the age of your child.

If you have not had a wellness visit in the past calendar year, we will ask you to make this appointment so we can complete both a wellness visit and a sports physical, plus ensure that your vaccinations are up to date.
